Welcome to 704aesthetics. These Terms & Conditions govern your use of our website, booking services, and the purchase and receipt of services provided by 704aesthetics. By accessing our website, booking an appointment, purchasing a service or product, or receiving treatment at 704aesthetics, you agree to these Terms & Conditions.

If you do not agree with these terms, please do not use our website or services.

1. MEDICAL & AESTHETIC SERVICES

704aesthetics provides aesthetic and medical services that may include, but are not limited to, neuromodulators, dermal fillers, biostimulators, regenerative aesthetic treatments, skin treatments, facials, chemical peels, microneedling, dermaplaning, laser or energy-based treatments, and other aesthetic services.

Certain services are medical treatments and may require a consultation and/or evaluation by an appropriately licensed healthcare provider before treatment.

Treatment recommendations are individualized. 704aesthetics reserves the right to decline or postpone treatment when a provider determines that a treatment is not medically appropriate, safe, or in the client's best interest.

2. CLIENT ELIGIBILITY

Clients must provide accurate and complete information regarding their medical history, medications, allergies, previous procedures, current treatments, and any other information requested by 704aesthetics.​ Clients are responsible for notifying their provider of any changes to their health, medications, pregnancy status, allergies, or other relevant medical information before treatment.

Certain treatments may have age restrictions or other eligibility requirements. 704aesthetics reserves the right to require identification and/or proof of age when appropriate.

3. CONSULTATIONS & TREATMENT PLANS

A consultation may be required before certain treatments. During a consultation, your provider will evaluate your goals, medical history, anatomy, and treatment options.​ A consultation does not guarantee that a particular treatment will be recommended or performed. Treatment plans may be modified based on clinical judgment, safety considerations, anatomy, treatment response, or other factors.

4. APPOINTMENTS & DEPOSITS

Appointments require a credit card on file and a non-refundable deposit to reserve the appointment. Deposits will be applied toward the cost of the scheduled service.

Appointment times are reserved specifically for each client. Clients are expected to arrive on time. Late arrivals, more than 15 minutes after scheduled appointment time, result in cancellation of the appointment. The applicable cancellation or no-show fee will apply.

5. CANCELLATION & RESCHEDULING POLICY

We understand that plans change. We kindly ask that clients provide at least 48 hours' notice when canceling or rescheduling an appointment. Appointments canceled or rescheduled with less than 48 hours' notice will be subject to a cancellation fee of $75.

6. PAYMENTS

Payment is due at the time services are rendered unless otherwise agreed upon in writing. We accept the payment methods listed by 704aesthetics at the time of service.​ Prices are subject to change without notice. The price quoted at the time of booking or consultation may differ from future pricing, promotions, or packages. Clients are responsible for all applicable taxes, fees, and charges associated with their purchase.

7. PROMOTIONS, SPECIALS & DISCOUNTS

Promotions, specials, discounts, packages, and promotional pricing are subject to their individual terms and expiration dates. Additional restrictions may apply to specific promotions.

8. GIFT CARDS

Gift cards may be subject to the terms and conditions provided at the time of purchase. Gift cards may be redeemed only for eligible products and/or services offered by 704aesthetics. Gift cards may not be exchanged for cash except where required by applicable law. Lost, stolen, or damaged gift cards may not be replaced unless otherwise required by law or determined by 704aesthetics. Gift cards expire after 1 year of purchase date.

9. RESULTS & EXPECTATIONS

Aesthetic and medical treatments do not guarantee a particular cosmetic outcome. Results vary from person to person and may depend on factors including age, genetics, anatomy, skin condition, lifestyle, treatment area, treatment technique, aftercare, and individual response.​ Before-and-after photographs displayed on our website, social media, or other marketing materials represent individual client results and are not a guarantee of results for any other person. Additional treatments may be recommended to achieve or maintain desired results.

10. RISKS & SIDE EFFECTS

All medical and aesthetic treatments carry potential risks and side effects. Depending on the treatment, these may include temporary redness, swelling, tenderness, bruising, discomfort, itching, sensitivity, asymmetry, lumps or bumps, pigmentation changes, infection, allergic reaction, scarring, or other complications.

Certain treatments may carry rare but serious risks. Clients will be provided with applicable treatment-specific information and consent forms before treatment. Clients should carefully review these materials and ask questions before proceeding.

11. MEDICAL EMERGENCIES & FOLLOW-UP

If you experience an unexpected or concerning reaction following treatment, contact 704aesthetics as soon as reasonably possible. For a medical emergency, including difficulty breathing, severe allergic reaction, significant vision changes, neurological symptoms, or other potentially serious symptoms, seek emergency medical attention or call 911 immediately.

704aesthetics is not an emergency medical facility and should not be used as a substitute for emergency medical care.

​

12. PHOTOGRAPHS & BEFORE-AND-AFTER IMAGES​

704aesthetics may take photographs for clinical documentation, treatment planning, and medical records.

​

Use of photographs for marketing, advertising, social media, websites, or other promotional purposes requires appropriate authorization or consent when required.

Clients may decline marketing use of their photographs without affecting their ability to receive treatment.

13. PRODUCTS & RETAIL PURCHASES​

Product recommendations are based on the information available to your provider at the time of recommendation.

Clients are responsible for following product directions and notifying 704aesthetics of any adverse reactions.

Retail product are all final sale.

14. WEBSITE CONTENT​

The content provided on the 704aesthetics website is intended for general informational and educational purposes only.

Website content should not be considered a substitute for individualized medical advice, diagnosis, or treatment.

Information on the website may be updated or changed at any time without notice.

15. ONLINE BOOKING​

704aesthetics reserves the right to cancel, reschedule, or modify an appointment when necessary.

16. MOBILE TERMS OF SERVICE​

Messages related to your account, orders, services, appointment reminders, marketing and promotional messages may be received. Frequency may vary.

You can cancel the SMS service at any time by texting "STOP." After you send "STOP," we will confirm your unsubscribe via SMS. After this, you will no longer receive messages from us. If you would like to opt back in, you may sign up again as you did previously.
